The accident investigation team found that Sanhe Shunxun Coal Mine also occurred on January 13 to conceal the accident, and then conducted an investigation and handling of the accident.

The report revealed that on January 13, a top -board accident occurred in the hidden surface of the mine, causing a coal mining team to die. A certified death certificate was issued in the hospital. After the body was cremated, the coal mine concealed the accident and reached a compensation agreement privately with the family members of the victims.

After the "2 · 25" accident, the State Council Security Committee issued a notice to implement the supervision of the investigation and punishment of the accident.

Less than a week interval, that is, on March 2, a coal and gas occurred in Limin Coal Mine, Qingzhen, Guiyang City, Guizhou Province, caused 8 people to die and concealed.

On March 24, the Safety Committee of the State Council supervised and assisted the team to enter the Guizhou Province to carry out supervision and assistance in production safety.

On August 26, the official website of the Guizhou Bureau of the State Mining Safety Supervision Bureau released a survey report of "2 · 25" major topboard accidents, which determined that this was a responsibility accident.

Sanhe Shunxun Coal Mine has illegal acts such as the stolen national resources, deliberately evading safety supervision, hidden illegal coal mining points, illegal contracting, illegal contracting subcontracting, unpopular top board management, and chaotic safety management.

The accident investigation team also determined that the Natural Resources Bureau of Zhenfeng County and the Industry Bureau did not perform their duties in place. There was a gap between the Natural Resources Bureau of the Southwest Guizhou and the Energy Bureau. There is a gap in the production safety of coal mine.

According to the investigation and the determination of the accident responsibility, in accordance with the relevant laws and regulations, 15 people including Yang Bing, an illegal contractor of the coal mine, were transferred to the public security organs to investigate and give administrative penalties. The agency takes compulsory measures; three people including Zhang Congjun, deputy general manager of Qianyue Company, and other people, it is recommended to give administrative penalties.

In addition, 39 people were suggested to give party discipline and government affairs. Among them, Liu Yixi, deputy secretary of the Zhenfeng County Party Committee and county head, was suggested to give serious warning and punishment within the party; Tu Yan, deputy secretary of the Southwestern State Party Committee and secretary of the Zhenfeng County Party Committee, was proposed to deal with it.

Born in February 1975, Tu Yan was the secretary of the Guizhou Provincial Party Committee of the Communist Youth League. According to public reports, in March 2021, she attended the public event as the deputy secretary of the southwestern state party committee (retaining the main hall), and later served as the secretary of the Zhenfeng County Party Committee.
