What conditions do I need to meet the fixed -point retail pharmacies of chronic diseases?

To declare a fixed -point retail pharmacy in chronic diseases, the pharmacy must meet the medical insurance service agreement that has signed a fixed retail pharmacy for three years, and the actual operating area has reached 80 square meters (inclusive). 16 conditions such as review areas, deployment areas, pharmaceutical service areas, and patient mission areas. Specifically:

1. In the administrative area of ​​Xi'an (including Xixian New District), a fixed retail pharmacy medical insurance service agreement has been signed for three years;

2. The actual operating area of ​​the pharmacy reaches 80 square meters (inclusive), and the chronic disease service area is set and there are clear marks. The service area should include prescription audit areas, deployment areas, pharmaceutical service areas, patient mission areas, etc.;

3. Establish and improve the internal management system and financial system corresponding to the medical insurance policy of chronic diseases;

4. Divided partitions and classification management of medicines and other products have obvious regional logos;

5. At least 2 professional (part -time) professional (and) personnel who are familiar with medical insurance laws and regulations and related systems are responsible for chronic disease management, and signed a labor contract for more than 1 year and within the contract period;

6. Equipped with pharmacists who have obtained qualifications for practicing pharmacist or have a professional and technical qualification certificate with pharmacy, clinical pharmacy, and traditional Chinese medicine, and registered in the place where the pharmacy is located. Provide pharmaceutical services for chronic disease participants;

7. With the requirements of information system construction that meets the needs of chronic disease management in our city, the system interface standards must be embedded, which can upload chronic disease settlement information and deposit data in real time;

8. Establish basic databases such as medical insurance and drugs, chronic diseases, and use national unified medical insurance codes in accordance with regulations;

9. Install the monitoring equipment, be able to monitor the drug service and medical insurance settlement in real time, and save the monitoring information records of no less than one year;

10. Improve the internal management system, have a complete drug sales and deposit management system, ensure that drugs enter the warehouse, outlet and inventory data in accordance with the number of drugs, tax tickets and lists with actual purchase of drugs, varieties, specifications, etc. The information is consistent;

11. Drugs supplying and reserves meet the needs of patients with chronic diseases. The drug regulations reach more than 2,000, and there are facilities and equipment that is suitable for drug storage conditions;

12. The price of drugs should be marked clearly to achieve integrity operations, standardized management, and convenient patients. Provide medical insurance services including: real -name system construction files, data review, cost management and control, drug guidance, etc.;

13. Pharmacy has been suspended from the service agreement without violations of medical insurance within one year. If there is no illegal behavior within three years, it has been punished by medical security and market supervision.

14. The quality and safety credit level of pharmacy pharmacies is "B" level or above;

15. The legal representative of the pharmacy, the person in charge of the enterprise or the actual controller of the implementation of the information disclosure network in China has no illegal and unscrupulous behavior;

16. Other conditions stipulated in the relevant laws and regulations of medical security.

Chronic diseases fixed -point retail pharmacies are declared according to quarterly. In the first month of each quarter, designated retail pharmacies fill in the "Application Form for Xi'an Medical Guarantee Chronic Diseases Pharmaceutical Store" and submit the application information to the medical security agency management agency managed by the agreement. On September 1, 2022, the application information of fixed -point retail pharmacies of chronic diseases began, and then accepted for quarterly. At the same time, the district and county medical security agencies will also organize an evaluation team or entrusting third -party institution to conduct inspections and evaluations in accordance with the "Investigation and Evaluation Form for the Pharmaceutical Reporting Pharmacy in Xi'an Medical Security Chronic Diseases". The assessment time does not exceed 3 months. After 3 months of evaluation, the application can be submitted again after 3 months. If the evaluation is still unqualified, it shall not apply again within one year.

The medical security agency will strengthen the supervision and management of fixed -point retail pharmacies of chronic diseases, innovate supervision methods, establish a dynamic management mechanism, and have a cumulative medical insurance agreement during the validity period. Those who are not in place, the incident of major pharmaceutical quality and safety incidents, and applying for improper means of fraud to obtain one of the 17 cases of the qualifications of fixed -point retail pharmacies in chronic diseases will be lifted, and the medical insurance service agreement of fixed -point retail pharmacy in chronic diseases will be lifted. Agreement, specifically:

1. If the medical insurance agreement has been suspended or above the medical insurance agreement or the medical insurance agreement during the validity period is not rectified or rectified as required;

2. An incident of major drug quality and safety occurred;

3. Apply for the qualifications of a fixed -point retail pharmacy with unfair means such as falsification;

4. Those who fake and alter the "enter, sell, deposit" bills and accounts, forgery prescriptions or the cost of insured personnel are deceived by medical insurance funds;

5. Replace non -medical insurance or other goods for medical insurance and drugs, sell medical insurance or use medical security funds;

6. For non -fixed -point retail pharmacies, fixed retail pharmacies or other institutions during the suspension of the medical insurance agreement are settled;

7. Transfer or gift medical insurance settlement equipment to others to change the venue;

8. Rejecting, obstructing or not cooperating with the medical insurance department to carry out intelligent review, performance assessment, etc., the plot is bad;

10. During the examination, the medical insurance department found that there were major violations of laws and regulations on fixed -point retail pharmacies of chronic diseases and may cause major losses of medical security funds;

11. If it is revoked, canceled the drug business license or business license;

12. If the administrative penalty decision made by the medical security administrative department fails to fulfill the administrative penalty of the medical security;

13. In violation of the relevant laws and regulations of the market supervision and management department, the quality and safety credit level of pharmaceutical companies is "C" level and below;

14. The legal representative, the person in charge of the enterprise, or the actual controller cannot fulfill the agreement on the medical insurance agreement, or violates the law and dishonest;

15. The fixed -point pharmacy actively proposes the termination of the medical insurance agreement and the consent of the medical security department;

16. Due to relocation, decoration, etc., you cannot provide medical insurance services for patients with chronic diseases for more than 6 months;

17. The agreement shall be lifted according to the medical insurance agreement.
