▎ Clinical issue:
Does the dose between drinking and biliary cancer (BTC)-Does the reaction relationship change due to different blood glucose status?
It is unclear whether the dose-response relationship between drinking and biliary cancer (BTC) [including bile duct cancer (CCA) and gallbladder cancer (GBC)] will change due to different blood glucose status.
A national queue study from J CLIN Oncol magazine shows that mild to moderate drinking in the early stage of diabetes and patients with diabetes is also related to the increase in the risk of BTC.
▎ Research plan:
This study was included in the national health screening, age ≥20 years, and cancer -free medical history of 9520629 in 2009. The progress of the BTC of these participants until December 2018, and the risk of regression analysis of the COX ratio risk regression analysis is used.
▎ Main discovery:
1) During the year -on -year follow -up of 78.3 million people, 21079 new BTC patients were issued.
2) In the early stage of diabetes and patients with diabetes, mild to moderate drinking can increase the CCA [Ahr), 1.20; 95%CI 1.13-1.28, AHR, 1.58; 95%CI 1.47-1.69] and GBC (AHR, 1.18; 95%Ci 1.07-1.31, AHR, 1.45; 95%CI 1.28-1.64) risk. In people with normal blood glucose, mild to moderate drinking has nothing to do with the risk of CCA or GBC.
3) When a large amount of drinking and diabetes coexist, the risk of CCA and GBC has increased (AHR, 2.04; 95%CI 1.83 to 2.26; AHR, 1.65; 95%CI 1.33-2.04; all P <0.001). Similarly, the risk of diabetes and a large amount of alcohol in the early stage of diabetes and a large amount of alcohol also have a synergistic interaction with the risk of CCA and GBC (all P <0.001).
▎ Looking out:
The study shows that in the early stage of diabetes and patients with diabetes, even mild to moderate drinking is related to the increase in the risk of BTC, but in people with normal blood glucose, no risk of drinking and BTC diseases have not been found. Fully avoiding drinking may help reduce the risk of BTC diseases in patients with diabetes and diabetes, which suggests that in the future, it is necessary to formulate personalized strategies to prevent BTC.
