2. Determine objects and scope

3. Related condition standards

Primary and secondary school senior teachers, senior lecturers of secondary vocational schools, senior experiments, and special teachers cannot be used as a basis for test -free.

(4) Physical examination standards. Strictly shall be implemented in accordance with the "Medical Examination Standards and Measures for the Application of Teachers' Qualifications" (Teacher [2002] No. 3).

The medical examination time is valid within 2 months from the date of the announcement of this announcement; the applicant will be selected from the list of medical examination hospitals (see Annex 1) from the Wuhan District Education Qualification Hospital for medical examinations, and use the medical examination form of the personnel qualification application personnel in Hubei Province. Fourth, teacher qualification identification steps

2. The original conclusion of the medical examination is the original "Hubei Provincial Teacher Qualification Application Personnel Physical Examination Form" (the medical examination form is not used, the medical examination is not effective within the designated hospital and the prescribed period of time).

4. 2. The original and photocopy of the "Putonghua level testing level certificate" shall be subject to the inspection of the national Mandarin training test information resources network certificate. No acceptance.

3. The original and photocopy of the qualification certificate of the qualification examination of primary and secondary school teachers, which are subject to the inquiry of the China Education Examination Network Certificate, and those who cannot check them will not be accepted.

(5) Teacher qualification identification applicants can only apply for a kind of teacher qualification within the same year. Please apply for online declaration and on -site confirmation according to the prescribed time, place, and requirements. For reasons such as error or incomplete materials submitted by information, if the declaration or supplementary materials are not completed within the prescribed period, the identification agency will no longer accept it; during the online report, the applicant should read the online report in detail, register the authentication as required, and fill in the report really accurately and accurately. Information, the platform cannot be verified due to the wrong report and concealment report, and the consequences caused by the applicant should be borne by the applicant.

(6) After the "Ideological Town Appraisal Form" is replaced by the "Personal Promise", the applicant has no criminal record certificate.
